Modifications to Local and National Bankruptcy Forms

Certain local and national bankruptcy forms have been modified to correct errors and conform them to changes in the law and other forms.
Practitioners should ensure that they are using the most current version of these forms.

Two local forms have changed. Previous versions of these forms are available in the archive.

Motion to Modify a Confirmed Plan for Chapter 13 cases

Four national forms have undergone minor editing:
B423 Certificate About a Financial Management Course
Certification to Court of Appeals
A Summary of Your Assets and Liabilities and Certain Statistical Information (individuals)
Bankruptcy Petition Preparer’s Notice, Declaration and Signature

Certain national forms will be changed automatically on April 1 under the provisions of Section 104 of the Bankruptcy Code.
